I think that the note was put there because it is not intuitive to feature the word for "pregnant" as a headword, because on its own it seems to refer to a pregnant man, which the mind finds ridiculous. The note seems to exist to justify or excuse the standard practice of having the lemma form in the masculine gender, because it generates a discordant image in the mind when it is considered prima facie. But in practice the masculine gender can be very common as applied to words like belly. I thought that the note did not ring unquestionably true enough to me, and I thought it was better to feature a common use example where "pregnant" will be masculine. Currently I feel an intuitive objection to saying that the feminine gender is "much" more common, because it has not been in my life, and possibly no value to a note at all, when the excuse is made by a use example, which shows that the masculine gender is still in understandable common use as a colocation.
