Apparently this is real (although I've never heard it in 30 years of living in Germany as a native speaker):

  • the German band "Die Ärzte" did record a song by that name
  • the "German missions in the USA" did run a story on it [1] and
  • it is in the Duden at [2] albeit with the least helpful explanation imaginable: Simply referring to the word Ohrfeigengesicht, which is not in the Duden.

That said, I find it important to point out: Both the Backpfeife and the Ohrfeige are slaps to the face (to the cheek and the ear, respectively) that are delivered with a flat hand, the desired effect being a loud slapping sound and irritation rather than injury, i.e. no long-lasting effects (the imagine that comes to mind is a father slapping his son and not two adults fighting). Any reference to fists (as in both the "German missions in the USA" article and in the current version of this page) is thus inappropriate in this context.
